Physiology of Ejaculation

Ejaculation is the emission of semen to the exterior, resulting from the contraction of muscles surrounding the male internal urogenital ducts (Medline's definition). It is closely connected to the male orgasm. Sexual behaviour has components of rewarding behaviour, and ejaculation appears to be the most reinforcing component. Little is known about the neurochemical basis of this rewarding experience. Some relations to opioids and their receptors have been shown (Coolen et al. 2004). Ejaculation consists of two phases. In the initial phase, emission, smooth muscles of the vas deferens, the seminal vesicles and the prostate, as well as their secretions, are involved. At the end, the mixture of spermatozoa from the epididymis and the vas deferens with the secretions of the seminal vesicles, forming about 50 of the ejaculate, and the prostate, which secretes nearly the other half of the semen, is made available in the prostatic (posterior) urethra. The sperm progression in the seminal...

Antidiarrheals Intestinal Antiinflammatory Antiinfective Agents

Following the first report by Levi et al. (1979) on oligo-zoospermia during treatment with sulfosalazine, and the recovery after cessation treatment, a number of studies stated limited or no impairment of spermatogenesis in treated men, while 5-amino salicylic acid was suggested to lack this effect completely. There are, however, no RCTs which demonstrate this effect without doubt. An interesting explanation of the contrasting results was given by Niederberger (2002), who suggested that the deleterious effect of sulphasalazine might take place only after ejaculation and result in asthenozoospermia.

General Information

Escitalopram was efficacious in patients with major depressive disorder in short-term, placebo-controlled trials, three of which included citalopram as an active control, and in a 36-week study in the prevention of relapse in depression (5). It has also been used to treat generalized anxiety disorder, panic disorder, and social anxiety disorder. Results also suggest that, at comparable doses, escitalopram demonstrates clinically relevant and statistically significant superiority to placebo treatment earlier than citalopram. The most common adverse events associated with escitalo-pram include nausea, insomnia, disorders of ejaculation, diarrhea, dry mouth, and somnolence. Only nausea occurred in more than 10 of patients taking escitalopram.

Its getting harder every day

What purpose does the female orgasm serve The initial answer is obvious to make a woman feel good so she will want to engage in sex. The whole truth is not nearly as obvious as for the male orgasm, which is pretty much a requirement for procreation so it is quite easy to see why making this feel good could lead to an evolutionary advantage. There have been all sorts of theories as to what the evolutionary advantage (if there is one) to the female orgasm is. Some

Physiology and Pharmacology

Effects on sexual arousal and ejaculation occur through a central mechanism, because the drug can induce arousal even after genital anesthesia (144) and it reverses clonidine-induced inhibition of ejaculation (145,146). However, the mechanism of action of yohimbine on erectile function was not fully understood and its site of action was not conclusively identified until it was demonstrated few years ago that functional a2 receptors are expressed in the human corpus cavernosum (147). It is now believed that yohimbine exerts its erectogenic action by blocking post-synap-tic a2 receptors in the corpus cavernosum and thus inhibits the contractility of the caver-nosal tissue (128). In addition to its a-adren-ergic effects, yohimbine exerts a stimulatory action on the mood and may increase anxiety. This effect is at least partially attributed to yohimbine's central serotonergic activity although it has not been adequately studied.

Problems In The Evaluation Of Clinical Results

Sexual experiences and behavior can be deeply influenced by the LSD pro-cess. The intensity, depth and completeness of the sexual orgasm and the ease with which it occurs seems to be closely related to the process of letting go of psycho-1 logical defenses. Many problems in this area can be traced back to unconscious confusion between the pattern of genital orgasm and that of the total physical release that characterizes the orgasm of birth. As LSD subjects learn to let go in the death-rebirth process, their orgasmic ability increases considerably this improvement of sexual experiences can be observed in both males and females. In those individuals who did not have any major psychopathological symptoms prior to the LSD session, the same effect can usually be observed after one or several high-dose psychedelic experiences. Sexual neuroses, such as frigidity, vaginal spasms (vaginism), genital pain during intercourse, impotence and premature ejaculation frequently respond well to LSD...

Hair Growth Disorders

The clinical dose of finasteride is well tolerated. The main side effects reported in phase III clinical studies were sexual function disorders including decreased libido, ejaculation disorders, and erectile dysfunction. All these sexual disorders were mild-to-moderate and were reversed on discontinuation of the drug, and in some patients, they resolved even with continued therapy (86). A 5-mgdose in hirsute women was well tolerated with no significant side effects, although the risk of developing abnormalities in the external genitalia of male fetus, if the drug is taken or a crushed tablet is handled by a pregnant woman, causes a great

Second Generation Effects Teratogenicity

Topical exposure of male neonatal rats to a commercial hexachlorophene formulation produced significantly reduced fertility, resulting in inability to ejaculate. It was possibly caused by dioxin, an androgenic'' contaminant of hexachlorophene, responsible for permanent disruption of the integrated ejaculatory reflex. However, in subneurotoxic concentrations, hexachlorophene did not seem to cause significant impairment of spermatogenesis in rats or dogs.

Ginseng and Copulatory Behavior Animal Studies

Panax ginseng and Panax quinquefolius have both been shown to enhance male copulatory behavior in laboratory studies (Table 4.1) 18-20 . Copulatory behavior can be examined in male rodents by placing them with sexually receptive female conspecifics and measuring the following parameters mount latency time from introduction of the female until the first mount with pelvic thrusting intromission latency time from introduction of the female until first mount with pelvic thrusting and vaginal penetration ejaculation latency time from the first intromission until ejaculation and postejaculatory interval or the refractory period time from ejaculation until next intromission. A reduced latency to mount the female would indicate increased male sexual arousal, whereas a decreased latency to intromit or ejaculate would be indicative of increased copulatory performance. In male mice whose normal copulatory behavior was compromised as a result of prolonged individual housing, the daily...

Cannabis in India ancient lore and modern medicine

Aldrich 22 documented the development of tantric cannabis usage around the seventh century as a mingling of Shaivite Hinduism and Tibetan Buddhism. Apparently, the 11th century text, Mahanirvana Tantra, is currently still consulted with regard to sexual practices, withholding of male ejaculation and promotion of sexual pleasure in both genders.

And Drugs

The ADEs on sexual functions are usually mild or moderate. They rarely prompt hospital admissions, which are classified as resulting from severe ADEs. This fact may explain why standard textbooks of pharmacology note sexual side effects of drugs only marginally. Entries of the 11 th edition (2005) of standard textbook Goodman & Gilman's Pharmacological Basis of Therapeutics can be quoted as examples of the unsatisfactory consequences of sexual side effects. Concerning the interaction of p-receptor antagonists with erectile function, which is considered an important cause of sexual dysfunction in elderly men, the textbook contains only two sentences The incidence of sexual dysfunction in men with hypertension who are treated with p-adrenoreceptor antagonists is not clearly defined. Although experience with the use of p-adrenoreceptor antagonists in pregnancy is increasing, information about the safety of those drugs during pregnancy is still limited Widerhorn et al., 1987 . Specific...

Sexual Dysfunction

Every class of antidepressants is associated with some degree of sexual dysfunction. Difficulties can include problems with libido, impotence, ejaculatory dysfunction, and anorgasmy. The incidence of sexual dysfunction is as high as 30 with SSRIs. As with other side effects, some of these may be symptoms of the depression itself, and they may predate the initiation of the antidepressant trial. Treatment can include dose

General

Sexual function Diuretics remain important in the pharmacotherapy of hypertension. Highdose diuretic therapy was the therapeutic norm in the 1980s, but in the 1990s a low-dose stratagem was instituted. With this change the incidence of diuretic-related electrolyte and metabolic adverse effects has fallen. Now the adverse effect of diuretics that is a major obstacle to their use is sexual dysfunction. The exact incidence is poorly documented, perhaps because of the personal nature of the problem and the reluctance of patients and or physicians to discuss it. The adverse effects of thiazide and thiazide-like diuretics on male sexual function include reduced libido, erectile dysfunction, and difficulty in ejaculating. These abnormalities have been reported with incidence rates of 3-32 . The true incidence of sexual dysfunction probably lies closer to the lower end of this range (1R). In a meta-analysis of 13 randomized placebo-controlled trials conducted over a mean of 4 years the NNH...

Och3

Despite its serious clinical shortcomings, guanethidine, which was introduced in 1960, still finds use in situations of severe or refractory hypertension, both essential and due to secondary causes (e.g., renal stenosis). Its main antihypertensive effect is now believed to be neuronal blockade by prevention of NE release from storage sites. Like the rauwolfia alkaloids, there is also NE depletion, but this only becomes significant with very large doses. Because the guanido group imparts such high basicity to this and other guanido-con-taining drugs (pKa 12), guanethidine is 99.99 protonated at physiological pH. It does not cross the BBB and, unlike reserpine, exerts no CNS-mediated sedation. Since it does not affect autonomic ganglia, it rapidly superseded the ganglionic blockers clinically and can be viewed as the evolutionary bridge to the newer drugs that followed. The high intensity of its effectiveness and its mechanism, however, are still responsible for its very disconcerting...

Finasteride

Finasteride is likely to be used less intensively in benign prostatic hyperplasia than in malignancies. In one study in 3040 men with benign prostatic hyperplasia the effects of fi-nasteride 5 mg day for 4 years were compared in men over and under 65 years (110C). In both groups the drug was effective and there were no significant differences in cardiovascular adverse events between placebo- and finasteride-treated patients. There were significant differences between placebo and finasteride in the incidence of typical drug-related adverse events, but there were no specific differences associated with age. The principal events were impotence (8.8 ), reduced libido (6.8 ), reduced volume of ejaculate (3.5 ), other ejaculation disorders (1.5 ), rash (0.6 ), breast enlargement (0.5 ), and breast tenderness (0.2 ). There were no drug interactions of clinical importance with finasteride.

Natural Stimuli

We recently measured faster dopamine transients in the core of the nucleus accumbens of male rats during sexual interaction (2). In the first experiment, we monitored dopamine during introduction of a receptive female into the test cage. Dopamine transients occurred during the initial interaction with the female, that is, approach and sniffing behaviors, but not during copulatory behaviors such as mounting, intromission, or ejaculation. Additional experiments addressed whether this phasic dopamine response is specific for recogni-

Mazindol

Mazindol increases pulse rate, may cause hallucinations, and disrupts sleep. In one study users complained of headache, skin rash, dry mouth, perspiration, tremor in heart and other muscles, nausea, and difficult urination and bowel movements. Users also report being wired, edgy, and dizzy. In human males mazindol can make the testes painful, interfere with erection and ejaculation, and cause urine retention. The latter effect has been exploited to treat incontinence. Mazindol may interfere with production of human growth hormone, a consideration when juveniles take the drug. When tested as a treatment for schizophrenia the drug at best had no effect and even worsened some symptoms. A medical journal article published in 2000 linked mazindol to pulmonary hypertension, the first time such an association was reported. Earlier reports noted development of heart disorder after taking mazindol in combination with fenfluramine or dexfenfluramine, but such affliction has been...

Ginkgo biloba

Clinical studies have also supported this effect. A double-blind, placebo-controlled study investigated the effect of 1,800 mg d dose of red ginseng in 90 men with ED 29 . Of this group, 81 men had psychogenic ED and 9 men had a mild vasculogenic cause for their ED. After 3 months of treatment, they found no change in patients with premature ejaculation, frequency of intercourse, and duration and frequency of early morning erections. However, men treated with ginseng showed an increase in libido in 50 of cases, greater sexual satisfaction in 43.3 , increased penile tumescence during erection in 53.5 , greater penile rigidity in 50 and decreased early detumescence in 23.3 (all p 0.05) (29). The authors gave the ginseng-treated group a therapeutic efficacy rate of 60 compared to 30 for the placebo group.

Sexual Disorders

Chart Drugs For

The recent extraordinary interest in sexual disorders was primarily spurred by the introduction of sildenafil (Viagra),the first effective oral medication for erectile dysfunction. The unparalleled global media hype that followed the launch of Viagra greatly increased the public awareness of erectile dysfunction and of sexual disorders in general. The enormous public response to sildenafil also stirred a debate over the reimbursement of such therapies and of lifestyle drugs in general. Several health authorities argued that impotence is not a disease and does not pose any health risk to theindividual or thesociety. To counter this argument, drug companies that are involved in the field of sexual disorders intensified their efforts in gathering evidence on the impact of sexual dysfunction and its successful treatment on the quality of life of the individuals and their partners. This gave a new life to other sexual disorders that, until recently, were overlooked by the public and...
